If you are a new client, please fill out forms sent to you through the patient portal (after booking your appointment) OR you can use the on-line forms below. The on-line forms must first be downloaded to your device, completed (don't forget to save your finished forms on your device), then emailed to me at tracyramble@themeritegroup.com
